The UCLA Faculty Group Dental Practice is comprised of a team of general practitioners, dental specialists, dental hygienists, and registered dental assistants. Together, the team discusses each patient's treatment options, allowing for efficient and coordinated care.
The FGDP offers dental services by UCLA faculty. In addition to providing general dental care, we offer services by Pediatric Dentists, Orthodontists, Implant Specialists, Prosthodontists, Oral and Maxillofacial Surgeons, Endodontists, Esthetic Dentists and Geriatric Dentists. All of these services are provided in one convenient location.

How to Make Appointments
Patients should call the receptionist at (310) 794-5750 to make an appointment. Patients can also e-mail the receptionist at FGDP@dent.ucla.edu.

What to Expect
In the first visit, the patient will be examined by a faculty member and radiographs if necessary, will be taken. If any services are necessary, the patient will receive a treatment plan documenting the services needed and cost of treatment. Cleanings will be scheduled after the first visit to the practice.

Request Dental Records
Patients should call our records department at (310) 267-4234 to have the records request forms mailed or faxed to them. Alternatively, a patient may download the form from our website. There may be fees associated with the duplication of the record.
